All Users of the Adult Activity Center are required to complete a 2016 User Form. Programs excluded from the User Fee are Elmview Senior Nutrition Lunch, Foot Care, Healthy Smiles Dental Cleaning Service, and Trips/Tours, which have a Non-User Fee. If you are unable to pay the User Fee, please talk with Katrina Douglas.

NEW! Each User will receive a key chain scan card. You will use your scan card when you sign in for activities.
